Frederick was born 10 Aug 1797 in Washington Twp., Franklin Co., Pennsylvania. He died 1846 in Woodberry Twp., Bedford Co., Pennsylvania.

In his father's will it is stated that Frederick was lame and unable to work. His will leaves to Samuel Burger "all that may belong to me out of the estate of my grandfather, Frederick Forman." He was not married.
